Office Policies & ProceduresThis page is provided for you to access the initial paperwork in advance of your first appointment. Prior to your first appointment, you will be asked to provide some basic information about yourself on an intake form, as well as read through the office and billing policies, a disclosure statement and a release of information. Please complete this in advance and bring it with you at the time of your first appointment, but you may also complete in the office when you arrive if you prefer. If you plan on completing it in the office prior to your first appointment, please arrive 15 minutes early to allow for enough time to complete the forms without taking up any of your session time.
